The "Fake it" Factor

In the course of suggesting help for people who have difficulty with hypnosis, one strategy that has come up fairly often is the "fake it till you make it" approach. The way I have seen it described is to simply follow the instructions given, whether the subject believes anything is happening or not, and over time, what was "faked" will become actual response.

Question: how does the hypnotist know when the subject is truly responding to suggestion, and when the subject is "faking"? Then, assuming faking is going on, can the subject's motives be determined, ie, is the subject using the fake it till you make it method, or just playing with the hypnotist?

Lastly, if it is determined that faking is going on, what is the proper response on the part of the hypnotist?

This depends largely on what motivates the hypnotist. If they actually care about working with the person to get them into a state of hypnosis, the proper response will be to switch strategies. As an example, instead of having you close your eyes they will switch it up and have you focus intently on an object.
