Panic attacks and orgasms

I'm struggling with a lot of things related to depression, anxiety and paranoia but I don't want to be attention seeking
One thing on my mind which I don't see mentioned a lot is that whenever I orgasm I have a panic attack and immediate feelings of self harming as in wanting to dig my nails into myself or just to kill myself
Everything comes to the surface and I can't handle it
