Worry comes from not accepting responsibility. Doesn't matter if it's not your fault. Say "I am responsible!", "I am responsible!" and you will feel more calm and confident.

Perfectionism is a problem when you don't take any action. You have to cut a big task into smaller sub-tasks and then work on 1 sub-task at the time. This way you won't be overwhelmed trying to make everything perfect in this major task. Perfectionism also lead to fear of failure (as you mentioned) ...
